You’ll receive a brief intake survey by email. Complete it before the call or together with your health coach — whichever works for you.

Meet your
health coach

Your LyfeMD health coach will learn about your needs and connect you with the provider best suited to your goals — a dietitian, therapist, kinesiologist, or coach.

Have your
Patient ID ready

You may be asked for your Patient ID number during registration. You can often find it in your original Patient Support Program email.
